About
367 Thornaby Road is our small residential home for adults with learning disabilities and associated support needs which we run as near to family principles as is practical.
People staying with us have individualised plans of care that enable their independence and ensure their health and wellbeing and quality of life.
Our team
Our team of staff is set up to support the needs of people staying with us at 367 Thornaby Road.
Our team includes:
- qualified nurse as registered manager
- nursing associate (each person has a lead nursing sssociate)
- health care assistants (each person has a key worker)
- admin
- house keeper
- volunteers.
Our facilities
Our purpose-built bungalow has:
- five bedrooms (four are en-suite) which people can decorate to their own personal style
- bedrooms are fully adapted to meet the physical needs of people in our care
- two of the bedrooms have direct access to a specialist bathroom suite with overhead track hoisting
- large dining room
- spacious and enclosed garden
- two lounges, one of which is also used for activities, including light and sound and interactive technology.
People staying at 367 Thornaby Road can go to local shops and facilities within the local and wider community such as swimming, rebound and trips to the theatre with our staff. All activities planned on an individual person centred approach which promotes quality of life and choice.
We also offer regular holidays and longer outings wider that than the local community
Meal times
Meal times are very important to people and home cooking features largely in daily life with a healthy lifestyle focus based on likes and dislikes.
Visiting the unit
Family / carers
Due to COVID-19 restrictions, all visits are facilitated outside of the home either in the garden area of prefabricated units. PPE and regular cleaning is in place to ensure safety following our Trust’s, and the local authority’s, guidance.
Estates / health and social care visits
Lateral flow tests are offered to all non-Trust staff before entering the home to ensure safety. All visitors sign in and out of the home on arrival.
To ensure all maintenance and repair work is completed safely and to limit the distress to the residents from noise all maintenance is planned to support the home and residents’ needs.
Support for parents and carers
All parents and carers are included in the assessment, planning and review of care for their relative. This is through links with keyworker groups, multi-disciplinary team (MDT) and within the annual reviews.
The home also follows the principles of Triangle of Care and regularly asks parents / carers for their feedback to continually assess the level of care and support offered to the families and include them in their relatives’ care.
How to access the service
The people who live at 367 Thornaby Road have done so for a number of years with no plans to move on.
If a vacancy did occur then the local care management team would make a referral to the service.
The manager would consider the needs of the new person and also their compatibility with the people already in the house.
