When we receive a referral, a qualified member of staff will complete an eligibility assessment to ensure that the person who has been referred meets the admission criteria. We may ask other professionals and services involved in the person’s care for more information to help us make the right assessment.
When eligibility criteria are met, we will offer a phased introduction to the service, at a pace to suit each person’s needs, including:
- meeting the staff at the unit
- developing a care plan
- setting up a series of short visits eventually leading to an overnight stay.
We then tailor short term care at regular intervals to suit the person’s own needs and the needs of their family.
