Providing community mental health services for people with dementia and older people. This service operates in Harrogate and surrounding rural areas.
Who is the crisis and home treatment service for?
The service assesses:
- people aged 65 and older who may be experiencing a mental health crisis.
- people of all ages if they have suspected or confirmed dementia.
Where will I receive assessment and treatment?
We aim to support and treat you in your community and aim to avoid admitting you to hospital where possible.
The service covers Harrogate and surrounding rural areas.
It operates:
- in the community, including people’s homes
- in care homes.
What support does the service provide?
The service provides a period of short-term, intensive support. The aim of our initial assessment, and following support is to:
- reduce any acute distress you’re feeling
- minimise your risk of immediate harm
- improve your mental health and your ability to complete tasks.
Support for people on hospital discharge
We provide support for people leaving mental health inpatient wards. You may receive support from the service when you return to your community after a hospital stay.
This can help with:
- timely discharge from hospital
- safe reintegration back into your community.
About the team
We are a team of multi-disciplinary professionals working in the community.
You may hear us referred to as CHTT. This stands for crisis home treatment team.
The team consists of and has access to:
- Medics including consultant psychiatrists
- Registered nurses
- Healthcare assistants
- Allied health professionals including occupational therapists and physiotherapists
- Psychology services
- Administrative staff
What to do in a mental health crisis
If you mental or emotional state gets worse quickly, this can be referred to as a mental health crisis or a mental health emergency.
In a mental health emergency, you can contact your local crisis resolution and intensive home treatment team by calling NHS 111 and selecting the mental health option, 24 hours a day, seven days a week.
